Super Duper has a really great case history form. Other sites do as well. Nothing really fit perfectly for what I need at the moment, so I made my own!
I lined up my parent case history surveys to align with the IEP. I know IEPs vary from district to state to region, but what I placed on the form, I feel, is pretty relevant all around to SLPs.
The form has four parts. First is a background & developmental history section where parents can share their preferred method of contact during school hours, family living situation/family speech history, language background, previous speech or related services assessments/therapy, and how they feel about their child's speech. Also under this section is a place where parents can share what their child's interests, preferences, strengths and weaknesses are.
Under health and developmental history, information about pregnancy, remarkable health history, and developmental milestones are discussed.
The third section is where social emotional, behavioral, vocational and adaptive living skills go.
Section four is for additional comments and information. You could leave this page off if you don't find it necessary. (I double-side copy, so I may sometimes just leave it off!)
